that those residing in the State of Ohio can use to join the Ohio Speedskating Association

1.  Applicant must be a resident of the State of Ohio
2.  Cost is $10.00 per person/max. $30.00 per family
3.  Form must be printed out, filled out ( it can't be filled out on line )
     and sent with a check made out to the Ohio Speedskating Association (OSA) to:
